For each question, choose the percentage of the time the experience happens to you when you are NOT under the influence of alcohol or drugs. 0% = never; 100% = always.

  1. 1Finding yourself somewhere and having no idea how you got there.
  2. 2Listening to someone talk and suddenly realizing that you did not hear part or all of what was said.
  3. 3Finding yourself in a place and having no idea how you got there.
  4. 4Finding yourself dressed in clothes that you don't remember putting on.
  5. 5Finding new things among your belongings that you do not remember buying.
  6. 6Being approached by people you do not know who call you by another name or insist they have met you before.
  7. 7Feeling as though you are standing next to yourself or watching yourself do something, as if you were another person.
  8. 8Being told that you sometimes do not recognize friends or family members.
  9. 9Finding that you have no memory for some important events in your life (e.g., a wedding or graduation).
  10. 10Being accused of lying when you do not think you have lied.
  11. 11Looking in a mirror and not recognizing yourself.
  12. 12Feeling that other people, objects, and the world around you are not real.
  13. 13Feeling that your body does not belong to you.
  14. 14Remembering a past event so vividly that you feel as if you were reliving it.
  15. 15Not being sure whether things you remember happening really did happen or whether you just dreamed them.
  16. 16Being in a familiar place but finding it strange and unfamiliar.
  17. 17Becoming so absorbed in a TV programme or movie that you are unaware of other events happening around you.
  18. 18Becoming so involved in a fantasy or daydream that it feels as though it were really happening to you.
  19. 19Being able to ignore pain.
  20. 20Staring off into space, thinking of nothing, and not being aware of the passage of time.
  21. 21Talking out loud to yourself when you are alone.
  22. 22Finding that you act so differently in different situations that you feel as if you were two different people.
  23. 23Being able to do things with great ease that would usually be difficult for you (e.g., sports, work, social situations).
  24. 24Not being able to remember whether or not you have just done something or perhaps had just thought about doing it.
  25. 25Finding evidence that you have done things that you do not remember doing.
  26. 26Finding writings, drawings, or notes among your belongings that you must have done but cannot remember doing.
  27. 27Hearing voices inside your head that tell you to do things or comment on things that you are doing.
  28. 28Looking at the world through a fog so that people and objects appear far away or unclear.
